Form T010 - Application form for registration of an industry code under Part 6 of the Telecommunications Act

This form should be completed and provided with a telecommunications, telemarketing, e-marketing or fax marketing industry code which is being submitted by an industry body or association for registration under section 117 of the Telecommunications Act 1997.
